IOWA BOARD OF EDUCATIONAL EXAMINERS Instructions for State and National Criminal History Record Checks 1. Please note that FINGERPRINTING MUST BE PERFORMED BY AN AUTHORIZED FINGERPRINTER. You will have to contact your local police department or sheriff’s office to set up an appointment to have your fingerprints taken. 2. Complete all appropriate items on the fingerprint card. Please print legibly. Be sure to complete the boxes labeled: Residence (Address) Employer and Address Reason Fingerprinted (Iowa Teaching, Paraeducator or Coaching License) Name Social Security Number Sex Race Hgt. (Height) Wgt. Have fingerprints rolled in appropriate boxes. 4. DO NOT FOLD, STAPLE, BEND, TAPE, or MODIFY the FBI fingerprint card. 5. Sign and date the WAIVER form. Include this form with the fingerprint card. 6. In Iowa, a deferred judgment is not considered a conviction once the defendant has been discharged after successfully completing probation. However, it should be noted that a deferred judgment may still be considered as an offense when considering charges for certain specified multiple offense crimes, such as a second offense OWI. If a disposition reflects that a deferred judgment was given, you may want to explain further to the Board on a separate piece of paper regarding the situation. 7. In Iowa, a deferred sentence is a conviction. The judge simply withholds implementing a sentence for a certain probationary period. If probation is successful, the sentence is not carried out. 8. The Board of Educational Examiners anticipates providing applicants with a “temporary permit” to individuals who have met licensure requirements, but have not yet cleared the FBI/DCI criminal history background check. This permit will allow applicants to teach, substitute, and apply for teaching jobs. It is anticipated that background checks may take up to four weeks for processing. 9. Send your fingerprint card, signed waiver, and check for $42.00 to the Board of Educational Examiners with your application. ** Note: The applications for new graduates from an Iowa institution do not need to include the application.
